Use the format below to develop a leadership SMART goal for YOURSELF, not your organization or department. The goal needs to be related to ONE of the Institute of Medicine’s (IOM) quality initiative, which includes five core healthcare profession competencies. However, your assignment focuses on only two of the initiatives. These are Patient-Centered Care or Working in Interdisciplinary and/or Interprofessional Teams. One of these competencies will serve as a framework for identification of your leadership goal. NOTE: Your two choices are Patient-Centered Care or Working in Interdisciplinary and/or Interprofessional Teams. SMART Goal Format: S—Specific (Who, besides YOU is involved in your goal, what is YOUR goal, and where will it take place?) M—Measurable (How are YOU going to achieve the goal?) A—Attainable (What resources and/or experts are available to assist YOU with attaining your goal?) R—Realistic (Is YOUR goal something that is realistically obtainable by YOU in YOUR professional practice? Explain.) My threaded discussion about SMART goal below and above is professor feedback Setting goals and attaining achievements is a necessary skill for anyone who works within the healthcare field. Goals that are patient centered "improve patient satisfaction and quality care" (Finkelman, 2012). According to this weeks lesson a SMART goal is specific, measurable, achievable, result focused, and time bound, all of us at some point in our careers have made SMART goals without even realizing it (CCN, 2016). The Institute of Medicine (IOM) discusses five competencies that are fundamental for all health care professionals. All are important but I will be focusing on Managing patient centered care utilizing informatics. My leadership SMART goal is based on Patient-Centered Care utilizing Informatics. This involves the importance of implementation of electronic documentation on a new system (Cerna). As a leader this implementation is instrumental in promoting change and allowing more specified patient-centered care and more time providing direct nursing patient care while utilizing the new Cerna system. My goal is that I will be trained as well as other staff members, to take care of patients and be able to complete all required documentation by the completion of the 12 hr shift. My advanced training will help my leadership development as a "Super User" for the new system and help staff members. S- Specific: I will be a instrumental resource for other nurses in the unit, as I will be trained and certified as a "Super User" for the new electronic documenting system (Cerna), by Dec. 2016. My focus will be learning the ins and outs of the new documenting system (Cerna) so I may be a helpful resource. I will after completion of training and certification work closely with other staff in order to ensure patient care and all required documentation be completed by shift end. M- (Measurable) I will be obtaining training sessions for 10wks, 4hrs a day, 3 days/wk to become a certified Cerna "Super User". This will be the 1st training to take place but thereafter will be offered 3 times per year, I am registered to start September 2016. I am very excited about this new experience learning new leadership skills. A- (Attainable) I will need to attend training sessions 3 days a wk for 4 hrs x 10wks. During this time the charge nurse for the shift or per diem staff will be covering my assignment R- (Realistic) I will attain this goal within the 10 wk timeframe allotted by my manager/director. I will be a certified "Super User" and efficient in documentation while maintaining direct patient care. Implementation of this new system will allow me to be the go to person demonstrating my new leadership skills, educating staff and new employees. T-(Time) My training will start in September 2016 and will continue over the course of 10 wks. The goal is that over the 10 wk period, my efficiency with the new system documentation will improve over each week. At five weeks (half way through training) I am to review and understand my weaknesses and work toward becoming more efficient. By week 10 I am to be very knowledgable about the system and competent to test for certification.
