Setting and Access
Over the course of twelve weeks, the goal of the program would be to decrease the number of people dying of congestive heart failure (CHF) or being hospitalized due to heart failure by about 10% using a smartphone-based program. Heart failure survivors, would be the target demographic. The team consists of project manager, other medical practitioners, and other personnel who will receive training. Financing will come from non-governmental agencies, and the location of the headquarters would be a medical center of choice. The smartphone-based program would have a hospital setting that would have specialized outpatient services for the heart failure patients. That is, the program would have special features that would allow patients to communicate effectively with the care nurses and other team members regarding their recovery processes or any other emerging issues.
Timeline for The Project
The project will take an approximated duration of about 12 weeks. The duration is enough considering that it will be the first phase or a pilot study that would not include many healthcare facets. The program will focus on using elements are being applied in the institution to have a better compatibility with the existing treatment options and management strategies for CHF patients.
