Instructions: Review the appropriate rubric to make sure you understand the criteria for earning your grade.Create a PowerPoint to present your strategic plan for your chosen at-risk population. The PowerPoint should include all the following information. One option is to create a slide for each instruction below.
- Title slide (per APA format)
- State your community and give a brief introduction of your community as noted in your Community Assessment
- State your at-risk population and discuss the reasons the at-risk population was of interest to you. Why did you choose it?
- Identify a key stakeholder, an organization that would back your plan and give you support. Add a synopsis of the organization, mission statement, and goal statement.
- Use a key informant from one of your assessments and provide the credentials that qualify them as a key informant. Explain what information this person shared regarding your at-risk population.
- Share the perspective of several of your at-risk population discussion/s that support or provide a need/foundation for your strategic plan of care
- Carefully analyze your community and at-risk population assessment findings, and the three priority Community Nursing Diagnoses established during the Data Analysis phase in Workshop Five.
- Select one NANDA Community Diagnosis established during the Data Analysis, which will be the focus of this strategic plan. Provide evidence of the need for this plan using information and statistics from your Community Assessment.
- Review the Writing Goals and Objectives PowerPoint and create a SMART Goal to be met at the completion of this plan addressing all constraints: Specific, Measurable, Action-oriented, Realistic (Time and Resource/Cost).
- Present a realistic strategic plan by creating three objectives that will help reach your goal. Each objective must have an expected end date.
- For each of objective, create three or more interventions. You may put one objective and the interventions on a slide, the second objective on the next slide, and the third objective on the next slide if you wish. Be sure to be detailed in the interventions you will use to meet each objective. Do not start your interventions in the middle of the process.
- Create a timeline from start to finish of your strategic plan. Getting permission and starting to get things together takes time. This will most likely need more than 6 months to achieve.
- Be specific in how each objective will be evaluated. Each objective must be measurable and must have an expected end date.
- Project if you will be able to meet your SMART Goal or not. If not, what barrier/s might get in the way to completing this goal?
- Conclusion: Summarize the benefits that this change initiative will have on the chosen at-risk population and community. Briefly describe the long-term and future impact of this proposed change initiative.
- References - you must include at least three scholarly references, cited in the slides, following APA format. Review NUR-465 Resources.
