Key points from the case study that I would incorporate into my clinical practice include performing a comprehensive physical assessment, taking a full past medical/surgical history family and social history, ordering laboratory tests, and diagnostic scans/imaging to make an accurate diagnosis, rule out other possible medical conditions associated with presenting symptoms, and treat symptoms and condition effectively. Placing a patient in the center of care is crucial. Patients and family members should be included in patient care and decision-making. Provider plays an important role to ensure patients fully understand their medication conditions and their options for treatment. It is critical to assess the patient’s understanding of the diagnosis and treatment to increase patient satisfaction and enhance the patient’s adherence to the treatment regimen. Patients with a better understanding of their health conditions and treatment plans will have better health outcomes (Yadav et al., 2019). When a lack of knowledge and understanding or questions or concerns are noted in patients, providers can offer patients a thorough explanation and education on diagnosis and available treatment options for better understanding and compliance. As noted in the analysis of the case study, there are many important things to consider before initiating pharmacological treatment and changing lifestyle for self-management. The treatment option that works the best in treating the condition does not necessarily mean it will be the best option for others. Depending on the circumstances, one best option that works for others could not be the ideal treatment choice for others. Therefore, different options for treatment and plans of care must be explored and discussed with the patient and family.

Key points from the case study which I will incorporate into practice include utilizing the subjective, objective, ROS, and patient’s report simultaneously to investigate the problem, and then implement a plan of care. As we learned, Mary presented with a short term problem (back pain), but then was discovered to also have a long term and significant health concern (OP). It is important to keep in mind when seeing patients how multiple issues may present together; or even as atypical signs and symptoms which often represent another health concern. In Mary’s case, both the OP and VCF will require attention and treatment with a plan for both.

The significance of this case study shows how treating the older adult may include thinking about multiple problems at once, including both short and long term health concerns. Unfortunately, due to time constraints individual consultations are often directed towards managing a single condition (Junius-Walker et al., 2019). In Mary’s case, she presented with the acute back pain. Roller-Wirnsberger et al. assert, the complexity of geriatrics warrants a comprehensive and systematic consideration (2020). The provider must incorporate a variety of clinical considerations, mortality risk as well as a personalized clinical care approach. Overall, the functional performance in decision‐making is essential to care for the aging population (Roller-Wirnsberger et al., 2020).
