Discussion- Assessing muscuskeletal Pain

When it comes to ankle sprain it is usually caused by the inversion type twist of the foot, which leads to the pain and swelling mostly in the lateral ankle complex that is composed in the calcaneofibular, talofibular and posterior talofibular ligaments.  The  nerve root that is likely to be involved in the pain is the peroneal nerve that is often injured or compressed near the knee with will lead to foot paind and foot drop when the patient tries to move her leg("Ankle Pain," 2010). To test for the pain, one can bend, twist or conduct a direct hit on the patient’s ankle or the foot which could have injured the foot bones, the blood vessels, ankle joint, muscles, and the tendons which lead to the foot pain.  Based on the level of the patient’s pain, other symptoms that should be explored include tingling or numbness, radiating pain like radicular or sciatica, pain in the compression area like the low back and the neck, needs and burning sensation and weakness which is especially involving various activities. Other differential diagnoses would be Achilles tendinitis, Achilles tendon rupture and flexor halluces longus tendinitis(Amaha & Nozaki, 2016). Some physical examination to perform the ankle will include moving it side to side, pressing on the sprained area, twisting or hitting.

Independent nursing practice usually refer to actions that are  taken by a u nurse and will not require any direction or orders from the physician or the doctor, or actions that the nurse will take without a written order on what is needed. One of the most common independent nursing practices include teaching or encouraging a patient regarding various aspects that they will in in care or recovery, or teaching various forms of therapy.  In some cases, independent nursing practice can go as far as making minor diagnosis or administering medication (LaRusso, 2016). Also when the patient is in labour, the nurse may continue with the procedure of delivery without the presence of the doctor; all these forms parts of where the nurse will work independently. 

One of the major advantage of independent nursing practice is the kind of innovation that comes with the practice where these nurses are emerging as new professionals in different healthcare settings and the community health management.  In this way the nurse is not only limited to working in a hospital setting or a community healthcare center but can work as a consultant  medical practitioner for private firms, clients among other forms (Brown, 2017).  The role of the nursing practice has thus changed from the traditional professional registered nursing role to realizing other capabilities that the nursing profession can provide.  The major disadvantage of independent nursing practice usually lays with liability.  In most cases a nurse that works in a medical center or hospital, will often be protected from liability in the event that there is a cases of negligence or that the nurse had done some sort of medical errors during practice (Brown, 2017). However, for an independent nursing practice, the liability in wholly laid on the nurse and this could risk her losing the license. 

In conclusion independent nursing practice is a good thing as it encourages growth and allows the nurse to improve self-esteem for the abilities that they are allowed to discover about themselves.

In patient-centered care, various individuals such as the patient, receptionists, physicians, advanced registered nurse practitioners, specialists, hospital staff, ED providers as well as a physician assistant collaborate to ensure improved care (Schottenfeld et al., 2016). Daily, these individuals work together to deliver quality care. Every individual in the team plays an essential role. For instance, the reception is the first point of contact when the patient visits the health care facility. The receptionist is the gatekeeper, mainly to preserve the schedule of physicians. Sometimes the receptionist can also take other roles as ordered by the RNs, such as checking on whether a patient requires routine lab work before a visit, reminding the patient of particular preventive services requirements like immunization. (Lancaste et al., 2015) The RN possesses a specific set of skills and plays a critical role during patient care. The RNs are involved in roles like taking and monitoring the vital signs and taking blood samples as directed by the physician. Physicians are always prepared to manage the health care and medical requirements of patients. The leading roles of physicians include diagnosis, medication prescription, and planning of treatment.

In collaboration with physicians, specialists such as pharmacists play a crucial role in the management of medications prescribed to the patient by the physician. Other specialists, such as psychologists, offer therapeutic assistance while the nutritionists manage the nutritional needs of the patient (Schottenfeld et al., 2016). All these professionals, from the receptionist to the nutritionists, promote timely access to appropriate care, and connect the patient to the relevant health care resources. In my organization, the team collaborates well, and everyone knows his or her roles. The team can, however, be improved by increasing physician-patient communication to enhance compliance with the treatment regimen for better health care outcomes. The team can also be improved by enhancing the role of every individual and promoting communications among the team members as this will positively enhance patient safety, quality of outcomes, patient satisfaction as well as job satisfaction of the health care professionals and specialists (Lancaste et al., 2015). Besides, the team needs to collaborate with the community–based care coordinators to know the population of patients and ensure that patients receive the needed care services to maintain a stable, healthy population and minimize further expenditure on care services.

There is no doubt that nursing unions benefit not only nurses but also the entire health system as well.  A nurse union is an organization of nurses that forms to protect and advocate for the interest of nurses through collective bargaining on behalf of the members. For the APRNs (Advanced Practice Registered Nurse), the union means monitoring staffing ratios, management of nurse shortages, and an increase in pay (Archibald, 2017). According to Archibald (2017), unions mean raised the productivity for the employer with better training, less turnover, and extended tenure of the workforce. This paper highlights the role of nursing unions and collective bargaining for APRNs.

The role of nursing unions and collective bargaining for APRNs is multi-faceted. The nursing unions and collective bargaining in their collective bargaining for APRNs established standards, wages, and protection for APRNs while also working to protect the nursing profession’s essential feature, which is the provision of quality care to patients. The unions’ most targeted collective bargaining concerns wages (Gaines, 2019). Research indicates that union APRNs make higher wages than non-union APRNs due to the collective bargaining of the nursing unions. Besides, the non-union nurses receive wage benefits as a result of the actions of collective bargaining, implying that the nursing unions’ role encompasses the entire nursing profession, whether unionized or not.

Besides, nursing unions also develop standards concerning what a nurse’s role is in the field of healthcare. The unions protect APRNs from working beyond their scope as well as protecting patients from a nurse who would work outside their expertise or under unfair conditions (Archibald, 2017). Through collective bargaining, APRNs are allowed to have shifts and breaks, leading to better healthcare outcomes for both APRNs and patients (Gaines, 2019). Most important, nursing unions, through their collective bargaining, are charged with limiting the care offered by APRNs and fight for the skill sets of APRNs so that they can get proper wages and recognized job duties. Lastly, the unions also advocate for the increase in the number of APRNs in the field of nursing.

Health Policy

Health care professionals directly influence the quality and cost of health via their diagnoses, prescriptions, treatment, and orders. However, health care experts say that there is a shortage of health care professionals, such as nurses and physicians, as well as rural health. The shortage of nurses, physicians, and rural health is a worrisome trend, especially to health care executives. The shortage of nurses and physicians makes it problematic to operate a profitable medical facility while at the same time balancing patient care and developing a good work culture that motivates employee retention. It is, therefore, essential to assess a policy that can support the shortage of nurses, physicians, and rural health. 

Medicare and Medicaid Payments to Teaching Hospitals Program

One of the current policies that support the nursing shortage, physician shortage, and rural health is the introduction of the Medicare and Medicaid payments to teaching hospitals program. This policy ensures more equitable funding of health workforce programs to create a more balanced health delivery ecosystem ultimately (Nuss, Robinson, & Buckley, 2015). Currently, the large majority of federal funding for the education of health care professionals helps teaching hospitals’ training of physicians and nurses. Teaching hospitals are currently providing medical care to patients while offering clinical education and training to future and present physicians, nurses, and other health care professionals. This program also supports rural health because it ensures that there are enough nurses and physicians that can be deployed in rural areas. It is important to note that rural health is affected by the shortage of nurses and physicians. The federal government, via this policy, subsidizes teaching hospitals by paying for the direct and indirect costs of resident salaries and fringe benefits and teaching supervision by faculty physicians and nurses. 

Proposal to Negotiate to Support the Needs of the Country as a Legislator

As a legislator, it is crucial to always put the needs of the country first before anything. One needs to listen, think, and vote his or her conscience. When negotiating to support the needs of the people, it is essential to note that the people will not always agree with you, but they will respect you for thinking through the issues and reaching a common ground (Clarke, 2016). As a legislator, I would ensure that my legislature is responsive to public demands and needs and bring the opposing side to the table to negotiate for the needs of the country. The negotiation will involve exchanges of opinions and building consensus. In the proposes of negotiating for the country’s needs, it is crucial to be prepared to compromise, deal with opposition, and trade the legislation that supports the needs of the country so that others can buy into a settlement.

Most importantly, the negotiation to support the needs of the country will include enough input from citizens and organized groups in law-making decisions. Besides, I would ensure that the degree of partnership is reasonable and that the majority party possesses enough clout to ensure that things get done (Clarke, 2016). Moreover, it is also vital to ensure that the legislature is deliberative and allows for a give and take and the open exchange of ideas at all phases of the informal legislative process.
